You only need to enclose step 3 and 4 in the envelope. Then turn in that envelope at IMPACT on a Sunday night.

Please have all the necessary information filled out, do NOT hand this in without items completed and make us chase you down for these items. If and when you are accepted onto the team, you will receive an email stating you have been accepted onto the team. After that, you can create an online profile page that will significantly help you raise support for your trip.
